• Instructions for Submitting Your Request

  • CSL students, use this form to submit requests for assistance with:

    Type I: registration issues; i.e. a course is closed, error messages, etc.

    Type II: advanced advising needs; i.e. internship planning, career planning, concentration change, etc.

    Please limit your request to one issue at a time, and submit multiple requests if necessary. All requests are forwarded to Dr. Erin Mason, the CSL Coursework Advisor. You will be notified of the status of your request, typically within 2 to 3 business days during a regular academic quarter.
