Terms and Conditions
The following are the terms and conditions of participation in the UCR CAMP Resource Award
pending the award itself. Submission of this form indicates that you understand and agree to them. It also indicates that failure to comply with terms and condition could result in consequences ranging from reduction/loss of stipends to removal from CAMP membership.
By completing this form, you are acknowledging that you will return the aforementioned items in their original states (markings, ripped pages, unauthorized installed software, etc.); failure to do so will result in immediate full charge to your GROWL account for the items.
This award is issued to the applicant described herein; the resource may NOT be lent, sold, entrusted, or used by or to another person, or otherwise redistributed in any fashion.
You are required to return this resource on or before the agreed stipulated deadline. Failure to return the resource will result in full charge for the original price of the item and will be reported to the UCR Police Department.
I understand that my resource award will be rescinded or modified if I withdraw from the program or fail to meet all terms outlined in this agreement (which may result in financial penalties and/or termination from the program).
CAMP serves NSF-defined underrepresented minorities (Black, Hispanic, Native American, Alaskan Native, and Pacific Islander-non-Filipino) majoring in mathematics, engineering, and the sciences, who are permanent residents or U.S. citizens. CAMP is designed for students interested in graduate research careers (non-health career aspiring).
