    MISS State Contestant Paperwork Requirements

    For Miss Ohio 2025
  • MISS OHIO 2025 CONTESTANT!

Here is a list of documents and items required to enter the Miss Ohio 2025 competition.

PLEASE READ EVERYTHING CAREFULLY!! While many of the forms are used every year, there may be minor changes that could be easily overlooked.

ALL ITEMS MUST BE SUBMITTED ONLINE! For items that require a signature, you may be required to sign online electronically. For items that require a third-party signature, you may download, print, or email the document to the third party for signature and then upload it when submitting the required form.

When you submit a form online, you will receive an email with the information submitted.

For MISS Contestants from Local Competitions, you MUST include your Local Director info for each online submission.  Your Local Director will receive a copy of any form submitted online.

Some of the forms you submit will need to be reviewed and approved.  In these cases, you will receive an email of the form starting with EDIT in the subject line and the STATUS DETAILS in red at the bottom of the email.  Please read these emails ASAP since they may require further action.

You now have the option to save your partially completed form. You may then return to the form later to complete and submit. This also allows you to start the form on one device and complete it on another device. See below for step-by-step instructions.

If you have any questions, concerns, or find any mistakes, please email misspaperwork@missohio.org or call Susan Swinehart at 740-334-7117.

  • Saving Partially Complete Forms

If you start filling out a form and are not ready to submit the form, you may save the partially completed form with your data.  The data will be saved for 90 days.  Once you submit the form, the data will be cleared.  To save the form with info, follow these steps…

    • Click the SAVE button at the bottom of the form, next to the SUBMIT button.
    • When prompted to connect with Google, Facebook, or Create an Account or Login, look toward the bottom of the popup windows and click the link SKIP CREATE AN ACCOUNT.
    • On the next pop-up window, enter your email address and click SEND LINK.
    • You will get a pop-up confirmation that the link has been sent.
    • DO NOT CLOSE THE CONFIRMATION WINDOWS UNTIL YOU RECEIVE THE EMAIL. If necessary, you can use the RESEND email button.
    • The email you receive contains a unique link that will be needed to return to this specific form to complete and submit.

Another benefit of the SAVE feature is you can forward the link to your Local Director to review the information before submitting the form.  However, you, the contestant, are the one responsible for submitting the form.

  • Deadlines

We have staggered the due dates for materials needed between now and Miss Ohio Week instead of making everything due at the same time. This will help you focus on what’s coming up, when we need it, and allow you to submit the most up-to-date information in each case.

However, when we need it, WE NEED IT AND EXPECT TO HAVE IT!!!

Therefore, in addition to providing the judges with your photo, fact sheet, and community service initiative essay, we will also provide them with a report indicating what was submitted on time, what was submitted late, and how many days late.

Any delays or changes on our part that may cause late submissions on your part will NOT be reported against you.  We understand there are situations outside our control.

I UNDERSTAND I AM RESPONSIBLE FOR MEETING THE DEADLINES SET BY THE MISS OHIO SCHOLARSHIP PROGRAM AND THE JUDGES WILL RECEIVE A REPORT INDICATING ANY LATE SUBMISSIONS.
