The Delegation Fundraising and Outreach Activity Report Form will be utilized to accurately capture each delegation"s participation to ensure the delegation retains their annual accreditation. The Head of Delegation (or representative) should complete this report no more than 30 days following the approved fundraising or outreach activity.
