• Dog Adoption Application

  • 1. We reserve the right to approve or deny any application and the right not to disclose the reason.

    2. You must be at least 18 years old to apply for foster or adoption.

    3. We reserve the right to visit the home where the dog will be residing prior to foster or adoption.

    4. All family members living in the prospective home must meet the dog prior to foster or adoption.

    5. We do not allow any dogs to be chained, tied up, or locked in a garage or crated for 5+ hours.

    6. Our dogs are "family" dogs. We do not allow our dogs to be "outside only."

    7. Adoption donations allow Second Chance Dogs (SCD) to pursue our goals of vetting and rehoming dogs in need.

    8. Adoption fees are determined on an individual basis. Expenses for out-of-state transports and vet care are factored into the amount. Our primary goal is to find responsible and loving forever homes.

  • TERMS OF ADOPTION

  • 1. I / We commit to: Keep the dog in my personal possession and provide proper and sufficient food, water, shelter, grooming, and humane treatment always.


    2. Contact and get veterinary care immediately if the dog becomes sick or injured and to keep current on all vaccinations as recommended by veterinarian and law.


    3. Will have my dog(s) spayed / neutered: 3.a. Puppies and young dog(s) must be spayed/neutered by the age of 12 months unless advised otherwise, in
    writing, by a veterinarian and approved by the board. Written proof of spay/neuter must be provided by a licensed veterinarian within 30 days of procedure. 3.b. SCD will contact you to request proof of spay / neuter. If you can’t be reached, SCD will contact your veterinarian to confirm the information. 3.c. SCD reserves the right to remove dog(s) that are not spayed / neutered within the time frame determined on items “3.a.” & “3.b.”. 3d. New dogs are not to be taken to dog parks for several weeks. All new dogs must be on a leash when being walked outside off property. Your new dog should not be confined in a room behind a closed door.


    4. Obey all animal control regulations governing the area in which I live.


    5. Never sell, trade ownership, abandon, breed, or dispose of this animal in any way. Agree to contact SCD if I must relinquish custody and explain the reason. This includes release to family members, friends, etc. I will hold the dog and work with SCD in helping to find a suitable home that is agreeable to both me and to SCD. Relinquishing the dog to a shelter/rescue is a breach of contract and can result in legal action.


    6. SCD reserves the right to make inspections of the living conditions of the dog. If the conditions of the adoptive home are found to be unsatisfactory, I will surrender the dog to SCD.


    7. I will assume full responsibility for this dog’s actions, including any damage done by the dog.


    8. I understand the temperament, health, habits and physical condition of the dog cannot be guaranteed. I am in full agreement of these terms. SCD is held harmless and not liable or responsible for any damage, accident, or injury
    resulting from the placement of this dog into my home.


    9. The Adoption Donation for a dog will be paid at time of placement unless prior arrangements have been made. Within six months after adoption, (or later with veterinarian recommendation) and with proof spay or neuter from
    your veterinarian, we will refund $75 to you.


    10. If renting, I will show proof of landlord approval.

  • Our dogs are in private foster homes; all applications must be pre-approved before meeting the dog.

    Adoption fees are determined on an individualized basis. Expenses for out-of-state transports & vet care are factored into the amount. Our primary goal is to find responsible and loving forever homes.

    Once a "meet and greet" has been arranged, please bring:

    • Pet ID tag (with your contact info)
    • Leash
    • Collar
    • Personal check or cash for the adoption donation

    Special Notes:

    • Sometimes several people apply for the same dog. We make our final decision based upon which applicant we feel would provide the best "forever home" for the particular dog. With multiple applications of qualified loving people, we take into consideration matching the dog's temperament and personality with the people and lifestyle best suited for sharing their lives with the dog.
    • When introducing two new dogs at a "meet and greet", we require that both dogs must be on a leash and be under control for an introduction, side by side walk. Dogs are not allowed to meet head on, face to face, no matter how friendly they may seem.
    • Dogs that are fostered or adopted through any rescue group may sometimes have behavioral issues. We are willing to help you, to the best of our ability, with basic issues, adjustments and suggestions to make the new transition as easily as possible.
    • It is recommended that a dog never be confined alone in a room, behind a closed door. (S)he will want to get out. We suggest putting up a child's gate so the dog can still see out and won't feel so confined.
    • We are not professional trainers and cannot guarantee our suggestions will always work.
    • We offer consultations during the first 90 days of the adoption. After that time, special cases will need to be reviewed by the SCD Board of Directors. Each dog is an individual. You may also want to consult with a professional dog trainer in your area
